EATING WELL NUTRITION PROGRAM
TOUCH recognizes that a person living with a chronic illness is not able to address their health needs if they do not have access to basic nutritious food items.
In 2003 TOUCH establish our Eating Well Nutrition Program after we noted improved health outcomes from our members (clients) who were receiving weekly home delivered pantry items.
Our Eating Well Nutrition Program is based on the ADA Medical Nutrition Therapy model. Medical Nutrition Therapy focuses on improving the members education and understanding around issues of nutrition. Goals of our program include improving and maintaining health and increasing understanding of the link between eating habits and medical conditions like Diabetes, High Cholesterol and Obesity.
TOUCH’s Eating Well Nutrition Program provides the following services for our members:
- Nutritional Counseling/Education
- Home Delivered Food Pantry
MEDICAL CASE MANAGEMENT
TOUCH offers in-home Medical Case Management services for our Rockland County neighbors living with HIV infection. Our Case Managers are trained in helping people access the healthcare and social services offered to them.
SUPPORTIVE CASE MANAGEMENT
TOUCH offers in-home Supportive Case Management services to anyone living with HIV infection in Orange, Sullivan and Ulster Counties. Our Case Managers are trained in helping people access the healthcare and social services offered to them.
Our Case Managers are here to ensure that no one has to be alone as they live with a chronic illness.
Our Living Well Support Groups:
Our weekly supportive services offers a safe space where members can freely express issues in living with chronic health conditions.
Women Living Well:
Women Living Well Program provides a safe support structure for women.
Men Living Well:
Men Living Well Program provides a safe support structure for men.
